Hypogonadism

Hypogonadism is a condition in which the body does not produce enough of the sex hormone testosterone. This can occur in both males and females, but is more common in males.

Some key signs and symptoms of hypogonadism in males include:

  • Decreased facial and body hair growth
  • Development of breast tissue (gynecomastia)
  • Low libido and erectile dysfunction
  • Fatigue, poor concentration, and depression
  • Loss of muscle mass and strength

There are two main types of hypogonadism:

  • Primary hypogonadism occurs when there is a problem in the testes with testosterone production. This could be due to genetic disorders, infections, injury, or damage to the testes.
  • Secondary hypogonadism indicates there is an issue with the pituitary gland or hypothalamus in the brain. These glands signal the testes to produce testosterone. If they are damaged or impaired, they do not send the right signals to the testes.

Causes of secondary hypogonadism can include:

  • Tumors in the pituitary gland or hypothalamus
  • Radiation or injury to the brain area
  • Certain medications, steroids, or opiate drugs
  • Liver or kidney disease
  • HIV/AIDS or sickle cell disease

Diagnosis involves a physical exam, personal and family medical history, blood tests to check hormone levels, and possibly genetic testing or MRI scans.

Treatments aim to replace testosterone to normal levels. Options may include:

  • Testosterone injections, patches, or gels
  • Tablets often containing testosterone undecanoate
  • Human chorionic gonadotropin (hCG) injections to stimulate testosterone production

Managing side effects and ongoing monitoring of treatment are key. Untreated hypogonadism poses threats like osteoporosis, infertility, and heart complications in the long run. Catching it early makes treatment easier with good outcomes.
